Thank you for all the responses.  I have received e-mail messages from 
others requesting the same information.  The following is the list of 
publications with URLs.  Unfortunately, they are PostScript - I have 
been searching for converters that work (prefer MS Word) but nothing 
seems to work, so I have some fiends that have access to PostScript 
printers.
